Depends. If you cure yourself (with Falion in Morthal) almost immediately, Isren will allow you to rejoin Dawnguard. However, if you waited too long and levelled up your Vampire perks, you will notice that Dawnguard members will start to attack you (especially in towns). At this point, it is too late.
