Should I help or kill Madanach?

Killing Madanach is easier said then done. You have to kill him quitely so that the other prisioners won't rush to his aid. After he is dead, take the loot from his body, which will include a key to the gate just outside of his chamber.

Is it bad to kill Madanach?

Should I let Madanach live?

If you don't kill Madanach, you can follow him and his men after escaping Markarth. They will lead you to Druadach Redoubt, a Forsworn encampment and cave that is now friendly. Keep in mind that siding with Madanach will only cause the Forsworn in Druadach Redoubt to become friendly to you.

Will Forsworn be friendly?

The Forsworn are the most common type of hostile NPC encountered in the Reach. Encounters with them will not occur under normal conditions outside of the Reach. It is not possible to join the Forsworn, but you can side with them through a quest series and one area will become non-hostile.

Can you become Thane of Markarth if you side with the Forsworn?

Yes, to my knowledge there is only one quest where you side with the forsworn (the forsworn conspiracy) and it has zero impact on the rest of the game. Indeed.

Yes, you have to go to jail as a part of this quest. I tried resisting and everything to avoid it, but I found no way of getting around this part (You can go ahead and kill those corrupt guards if you want, you'll earn a bounty but you'll still go to jail if you walk out of the shrine).
